Donna Marie Chada, 54 years old, of Beaver Dam, passed away on Wednesday, June 29, 2016 at her home. No formal services will be held.Donna was born on April 9, 1962 to Melvin and Ruth (Wlodek) Chada in Milwaukee. She was a graduate of Hamilton High School in Milwaukee.Donna is survived by: her sisters & brothers – Elizabeth (Jerry) Peterson of Theresa, Julianna Diaz of Milwaukee, Barbara Chada of Albuquerque, NM, Diana Bieganski of Bruce, WI, Patricia Jelinski of Bruce, WI, and Catherine Chada of Milwaukee; nieces, nephews, other relatives and friends.Donna is preceded in death by her parents and her brother Dennis.
